Community Partnerships

Lorain County Public Health (LCPH) works with community members to plan, carry out, and evaluate community health initiatives. We partner with communities in various formats, such as community collaboratives, coalitions, committees, and boards. Working in partnership helps gather like-minded partners and resources to prevent disease and promote healthy behaviors.

LCPH lead partnerships:

Community Health Improvement Plan (CHIP) Steering Committee

  • Location: Lorain County
  • Goal: Work on a 3-year schedule to guide and implement broad-based, unified health strategies. Current Lorain County CHIP priorities include: Chronic Disease, Maternal and Child Health, Mental Health, Substance Use Disorder and Cancer
  • Accomplishments: The Community Health Improvement Plan was updated by team members and is available to review. Teams are making progress on specific strategies outlined in the document. 

Common VIEW

  • Location: Clearview, Firelands, and Midview School Districts
  • Goal: Ensure safer schools and positive social environments that improve both educational and social outcomes for students in the districts.
  • Accomplishments:
    • School-based food pantries
    • Shared-use for indoor walking and outdoor walking paths
    • Salad Bars in lunch rooms

Health Equity Task Force 

  • Location: Lorain County 
  • Goal: Improve health outcomes and equity for marginalized communities in Lorain County through partnerships with key stakeholders.
  • Accomplishments:
    • Conduct listening sessions with marginalized populations
    • Identified and offered equity training to LCPH staff and community partners 
    • Define and identify workforce pipeline resources 

Keystone Empowers You

  • Location: Keystone Local School District community 
  • Goal: Improve whole health and wellness through collaborative planning and action in the Keystone Local School District community. 
  • Accomplishments:
    • Blood pressure cuffs available for checkout at the library
    • Shared- use for indoor walking at Keystone Elementary School and LaGrange Village Hall 
    • Storybook Stroll at LaGrange Community Park
    • Fresh food pick-ups 
    • Outdoor walking at Penfield Recreational Park

Live Healthy Lorain

  • Location: City of Lorain
  • Goal: To increase active living and healthy eating opportunities in the city through policy, systems, and environmental changes. Supported by the Creating Healthy Communities (CHC) program at the Ohio Department of Health.
  • Accomplishments:
    • Falbo Park improvements: bike parking, water access, inclusive equipment
    • Tobacco-Free beaches at Lakeview Park
    • City-Fresh CSA program accepting SNAP/EBT for produce pick up at multiple locations  

Live Healthy Oberlin

  • Location: City of Oberlin
  • Goal: To increase active living and healthy eating opportunities in the city through policy, systems, and environmental changes. Supported by the Creating Healthy Communities program at the Ohio Department of Health.
  • Accomplishments:
    • Legion Fields Community Garden partner
    • Breastfeeding supports in the workplace at Kendal at Oberlin and CoWork Oberlin
    • Tobacco-free campus at Oberlin College
    • Connectivity, crossing, and bike parking improvement

Lorain Connected

  • Location: City of Lorain
  • Goal: To create and expand safe, accessible, and effective options for walking, biking, and busing throughout the City that are equitable and reliable for residents and visitors of all ages and abilities.
  • Accomplishments:
    • Lorain Public Library bike share, at Main & South Branches
    • Sidewalk, crossing, and bike lane improvements
    • Girls in Gear bicycle education program
    • Lorain Better Block

Lorain County Fetal Infant Mortality Review Board

  • Location: Lorain County
  • Goal: To review all fatal fetal and infant cases in Lorain County to identify trends and improve policies, systems, and environments to prevent future tragedies.
  • Accomplishments:
    • Community campaign in partnership with the CDC's Hear Her Campaign, Know the Maternal Warning Signs 
    • Education to Community Health Workers on maternal warning signs, home blood pressure monitoring, and prenatal care 
    • Plan of Safe Care initiative in partnership with OB -GYN providers to support individuals who are pregnant and experiencing a substance use disorder  

North Ridgeville Heart and Sole

  • Location: City of North Ridgeville
  • Goal: Improve whole health and wellness through collaborative planning and action in the City of North Ridgeville. 
  • Accomplishments:
    • Shared-use for indoor walking at Liberty Elementary School and LCCC University Partnership Ridge Campus 
    • Outdoor fitness equipment
    • Produce stand tours and fresh food pick-ups
    • Improved and enhanced crosswalk 

Safe Communities Coalition

  • Location: Lorain County
  • Goal: Reduce injuries and deaths on Lorain County roads through community initiatives that involve engineering, enforcement and education.
  • Accomplishments:
    • Buckle Up Bowl
    • Buckle Up for a Cup and Buckle Up for Burgers
    • Zero Proof Mix Off event

THRIVE! Southern Lorain County

  • Location: Village of Wellington and surrounding townships
  • Goal: Reduce the risks of obesity, heart disease ​and diabetes in the region through health eating, active living, and social/emotional support programming.
  • Accomplishments:
    • THRIVE! Turkey 5!
    • Chair Yoga classes
    • Indoor walking
    • Fresh produce pick-ups with local farmers
