Eligibility and Procedure for Prescription Assistance
Eligibility
Eligibility for the Drug Repository Program is in accordance with the 2001-2002 Ohio General Assembly’s House Bill 221 known as Karon’s Law. A Prescription Assistance Screening Tool is used by medical providers to determine eligibility for individuals requesting medications through the Drug Repository.
Procedure for Prescription Assistance
The following procedure is used to determine program eligibility for individuals requesting prescription assistance:
For Hospitals:

Screen emergency room and in-hospital patients using Prescription Assistance Screening Tool to determine eligibility and obtain signatures for all patients.

Fax completed screening tool, prescription(s), and cover sheet stating patient’s eligibility to Buderer Drug Company.

Fax completed screening tool and prescription(s) for all eligible patients to the Lorain County General Health District.
For Physicians’ Offices:

Screen patients using Prescription Assistance Screening Tool to determine eligibility and obtain signatures for all patients.

Fax completed screening tool, prescription(s), and cover sheet stating patient’s eligibility to Buderer Drug Company.

Fax copy of completed screening tool and prescription(s) for all eligible patients to the Lorain County General Health District.
For Clinics:

Screen patients using Prescription Assistance Screening Tool to determine eligibility and obtain signatures for all patients.

Fax completed screening tool, prescription(s), and cover sheet stating patient’s eligibility to Buderer Drug Company.

Fax copy of completed screening tool and prescription(s) for all eligible patients to the Lorain County General Health District.
After patient is deemed eligible, the prescription is filled by the Drug Repository at Buderer Drug Company in Sandusky, Ohio and mailed directly to the patient. A follow-up call is made to the patient by the Lorain County General Health District to assure receipt of the medication and determine other needs.
Notes:
-
Buderer Drugs will supply a list of available medications to the Lorain County General Health District on a bio-monthly basis or as changes occur.

Buderer Drugs will fill and mail prescriptions directly to address given by patient on screening tool within two days of receipt.

Invoices will be sent to the Lorain County General Health District for payment and mailing costs of prescriptions.

Lorain County General Health District is the fiscal agent for the Drug Repository Program.

Financial assistance for the Drug Repository is provided by the Community Foundation of Greater Lorain County.
-
Lorain County General Health District will provide public health nursing services to those within the health district that are identified as in need of services.
