Home Sale Inspection Program -- Are you buying or selling and existing home? This program is offered to help assist persons involved in the sale or purchase of a home that has its own sewage treatment system. Homes served by municipal sewers are not included in this program. It is often necessary when securing a loan to have the sewage system and/or the water system (well, cistern, etc.) inspected by the local health authority. This form can be completed and mailed to this Agency, along with the fee, to help expedite the process.
HSTS Abandonment Form -- If sanitary sewers are being installed on your street you will want to have your old system pumped out and collapsed to eliminate future problems. Please use this form during the process so that there is an official record of it being properly abandoned.
Notice of Intent to Fill -- This form shall be submitted when clean, hard, fill is being used for construction purposes or when a site is being brought up to grade.
Nuisance Complaint Form -- If you believe there is a public health nuisance condition in your neighborhood, please send a letter or complete and mail in this complaint form. We will schedule an inspection to see of there are any rules or regulations being violated.
Ohio EPA Sewage Treatment System Application -- New businesses or commercial buildings that do not have access to sanitary sewers require EPA approved systems for wastewater treatment. Use this information and application form to help explain and speed up the process. It should be submitted to the Ohio EPA in Twinsburg or to our health department.

Replacement Application -- Having problems with your household sewage system? The first step is to contact a Registered Sanitarian at this agency. He/she can advise you what the problem may be and what is needed to correct it. It may be necessary to submit this Application after a review of the property has been made.
Site Review Applications -- Planning on building a new home? Before buying a vacant lot in a non-sewered area of the Health District be certain that it qualifies for a sewage system (Health) Permit.
Subdivision Feasibility Applications -- Interested in developing a subdivision? The first step is to contact a Registered Sanitarian at this agency if municipal sewers are not accessible. He/she can advise you on the necessary steps to start the review process. It may be necessary to submit this Application after the Ohio Environmental Protection Agency has been contacted and conducted their review of the property.
